*As a part of our team, you will:*
* Identify environmental impediments to patient progress through interviews and review of patient records.
* Provide, via telehealth, counseling/psychotherapy services to patients.
* Be an active participant in SC/GA House Calls Chronic Care Management system and recording minutes of services provided for chronic care management of the patient.
* Assist providers and patients with resources to problem solve any issues related to care.
* Utilize EMR text and company email for intercompany communication with timely responses.
* Provide excellent customer service when communicating with patients, families, referral sources, and providers.
* Must be available during normal work hours (unless previously approved by direct supervisor).
Additional hours may be required to complete normal business functions and/or projects.
*Must Haves:*
You must be a compassionate and empathetic professional with the ability to provide the highest standards of care to our patients.
It is also important to have excellent communication and interpersonal skills and the ability to interact in a positive and motivating manner.
* Bachelors Or Masters Degree in Social Work or Social Services preferred.
* License in Social Work preferred.
* A minimum of one year of experience in a healthcare setting preferred.
* Previous experience in healthcare setting is preferred.
* Must be able to work flexible hours and travel between offices, facilities, and other places.
* Ability to utilize electronic documentation required.
Job Type: Full-time
Pay: $60,000.
