*Job description* *SCHEDULED HOURS*: 8:30AM - 5:00PM Monday through Friday (varies as needed by Administration *Job Description* Provides job development and delivery of vocational rehabilitation services to adults with mental illness.
Collaborates with other treatment providers and members of the community.
Follows the eight principles of the Individualized Placement and Support model for Supported Employment.
Engages individuals, health providers and natural supports when necessary, in an exploratory discussion about the individual’s interests, competitive employment and long term goals.
Provides and coordinates access to information about vocational services.
Emphasizes strengths.
Minimizes consequences, such as criminal history, periods of unemployment, etc.
when job developing with employers.
Engages employers to develop a relationship in order to match individuals on caseload with the needs of the employer.
Attend job fairs with the individual, and assist with creating resumes.
One-on-one services designed to assist the individual sharpen job-specific skills, task, responsibilities and behaviors on and off the job site.
Drives a SBHS vehicle during the day and transport individuals when necessary to and from interviews while developing transportation plan.
Provides and/or coordinates access to information about vocational services offered by GVRA/VR and assists in the application process and regular follow-up communication with GVRA/VR staff.
Determine if individuals receives SSI, SSDI, or other benefits which might be affected by an increase in income, and provide or coordinate access to informational resources about work incentives and benefits counseling.
Develop, with the individual and with his/her permission natural supports and/or existing records a profile.
Explores, identifies and documents desirable and suitable job types and work environments.
Ensures that the Vocation Profile is integrated into the individual’s EMR.
Provides direct or indirect effort on behalf to the consumer to integrate, coordinate, and reduce duplications of the individual’s SE service with TORS and other behavioral health/Vocational Rehab or other pertinent services.
Cultivates relationships with potential employers in order to explore and develop competitive employment opportunities.
Documents face to face contacts with the hiring manager, documenting in EMR.
Teaches, assists, and supports the individuals to emphasize their strengths and minimize their challenges.
*Minimum Qualifications* *_Bachelor’s degree in a social services related field_*.
At least one years of experience in behavioral health case management.
*Must have a valid driver’s license with a good driving record*.
Proficiency in MS Word, Excel, Outlook and Power Point software.
*Preferred Qualifications* Prior experience in documentation through electronic medical records.
