*JOB DESCRIPTION*: PURPOSE AND SCOPE Provide care coordination services using whole-health mentality.
Provide community engagement and coordination of services based on the individual needs of the person receiving services.
Ensure Care Coordination techniques and data are meeting CCBHC criteria.
DUTIES AND RESPONSIBILITIES a.
Provide support for a full array of behavioral health care, long-term community-based services and supports and primary and acute physical health services.
b.
Provides daily care coordination for members participating in the CCBHC Expansion grant.
c.
Coordinate services and activities with Designated Collaborating Organization (DCO).
d.
Provide education, as needed, to clients during medical appointments to assist in the understanding of disease knowledge, medication compliance, specialty services recommended by primary care provider and/or any other medical needs the patient may require assistance in understanding e.
Assist consumers and their families in problem solving potential issues related to the health care system, financial or social barriers (e.
g.
, request interpreters as appropriate, transportation services or prescription assistance).
f.
Assist consumers in securing medical assistance where needed through initiating the presumptive eligibility process.
g.
Facilitate psycho-educational groups as assigned by Program Director.
h.
Provide appropriate linkages, referrals, coordination, and follow-up to services and supports, including referrals and linkages to long term services and supports.
i.
Ensure and when necessary, provide health care appointment scheduling for mental health, substance abuse, and physical health providers.
j.
Identifies barriers for discharge and a plan to address them and ensure outpatient follow up care.
k.
Obtains collateral information and collaborates with clients' families, support systems, outside providers and involved agencies regarding progress in treatment and continuity of care.
l.
Serve as a community educator by providing mental health information to relevant individuals, organizations, and groups.
m.
Negotiate on behalf of the client, as needed, with the treatment team, and patients themselves by creating reasonable goals, clarifying the issues to be negotiated, following through with the agreement reached, while remaining flexible and consider culturally sensitive and holistic alternatives.
n.
Assists new CCBHC clients who are current clients in completing paperwork and releases of information needed.
o.
Provides system navigation and acts as a point of contact for CCBHC clients and families, with clients and families having direct access for asking questions and raising concerns.
p.
Assist clients with understanding the relevant details of the visit, as requested by either the client or the treatment team.
q.
Schedule appointments, as requested by the treatment team and explain purpose of appointments in clear and simple language.
r.
Participate as a member of a multi-disciplinary team including both medical and psychiatric members towards the development of treatment plans and implementation of services.
s.
Work closely with community resources and organizations in coordinating client services.
t.
Advocate for patients who may be vulnerable or need additional support.
u.
Provides leadership and models practices that support client centered care, family engagement, cultural competency, and resiliency and recovery principles.
ORGANIZATIONAL RELATIONSHIPS Report directly to Health Education and Care Coordination Director b.
Attend interagency and other provider team meetings to promote collaboration of service and treatment providers.
*JOB REQUIREMENTS*: _A.
_ _Qualifications, Education, and Training:_ a.
Extensive knowledge with regulatory, licensure, and credentialing bodies.
b.
Capable of carrying out a given task with all details necessary to get the task done well.
c.
Works well as a member of a group d.
Extensive knowledge of the principles of behavioral health treatment, psychiatry, counseling, and psychology.
e.
Knowledge of SAMHSA grant expectations.
f.
Organizational and time management skills; verbal and written communication skills; decision making and problem-solving skills _B.
_ _Competencies/Technical Requirements:_ a.
Experience providing care management services in a healthcare setting.
b.
Ability to utilize Electronic Medical Record Systems c.
Knowledge of Microsoft Suite Tools such as Excel, PowerPoint, Power BI, etc.
d.
Must have excellent written and verbal communication skills e.
Must be proficient on computers and highly organized _C.

_ _Expectations regarding quality and quantity of work:_ a.
Completes all agency and department required trainings.
b.
Completes the required grant reporting as outlined by SAMHSA c.
Ensure releases of information and other required documents are completed and filed.
d.
Engages partner organizations and reports grant progress and outcomes.
e.
Complete and file all documentation as required in a timely manner.
f.
Utilize Community Transition Plan form when needed.
Attends work as scheduled.
Follows all policies and procedures regarding dress code.
Follows all policies and procedures regarding professional conduct and ethics.
j.
Completes all required annual/bi-annual trainings as required through DBHDD and CARF.
Job Type: Full-time Pay: $38,000.
00 - $41,600.
