Company: Central EMS Position: Community Health Social Worker - CHIP Program Definition: Social Worker provides intake into the CHIP Program for clients needing assistance in navigating and accessing local healthcare resources to reduce inappropriate use of 911 Emergency Medical Services resources.
Social Worker in coordination with client, client’s family, clients primary care physician and local community health and social service agencies creates a care plan designed to improve client's health status while reducing inappropriate use of 911 Emergency Medical Services resources.
Social Worker conducts visits to enrolled clients’ homes to evaluate and improve their health status through improved access to local community health and social service agencies in accordance with client's care plan goals.
Social Worker graduate’s clients from CHIP Program as clients care plan goals are met, arranging for follow up calls and/or visits as needed.
Minimum Requirements: · Must possess master's degree in social work.
· Must possess license as a Licensed Independent Social Worker.
(Preferred) · Must have a minimum of two years community health and/or social services experience.
(Hospice, Home Health Care, Medicaid, Homeless Advocacy, Medicaid Enrollment, etc.
) · Must possess a high degree of personal integrity and responsibility.
· Must have solid verbal and written communications skills.
· Must be proficient with all Microsoft Office software packages.
· Must be currently licensed with state motor vehicle operator’s license.
· Must have a clean driving record as approved by company vehicle liability insurer.
· Must have the ability to work effectively with minimal supervision, using good judgment.
· Must have the ability to interact effectively and professionally with clients, client's family members, and the general public in such a manner as to reflect positively on the company.
· Must obtain certification in CPR by the AHA or other appropriate organization.
· Must obtain certification in First Aid by the ARC or other appropriate organization.
· Must obtain certification as Emergency Medical Technician within one year of hire.
Physical Requirements: · The ability to drive motor vehicle.
· The ability use mouse or keyboard to input data into computer.
· The ability to view graphic and text data on multiple computer screens.
Reports To: · Field Supervisor and/or Director of Operations (Operational).
· Medical Director (On all care plan development and follow up activities).
Specific Duties: · Provides intake into the CHIP Program for clients needing assistance in navigating and accessing local healthcare resources to reduce inappropriate use of 911 Emergency Medical Services resources.
· In coordination with client, client’s family, clients primary care physician and local community health and social service agencies creates a care plan designed to improve client's health status while reducing inappropriate use of 911 Emergency Medical Services resources.
· Conducts visits to enrolled clients’ homes to evaluate and improve their health status through improved access to local community health and social service agencies in accordance with client’s care plan goals.
· Graduate’s clients from CHIP Program as clients care plan goals are met, arranging for follow up calls and/or visits as needed.
· Maintains appropriate communications with company dispatch, supervisors, medical control and other agencies via radio, data link, pager or cell phone as required.
· Be knowledgeable as to local geography to include location of hospitals, public venues as well as local streets and highway systems.
· Fully and completely documents all program activities in client’s care plan file.
· Adheres to Universal Precautions/Body Substance Isolation when dealing with patient contacts.
Is familiar with and practices the Infection Control policies and procedures of company.
· Performs duties related to public relations and community education programs sponsored by company.
· Accepts responsibility for recertification/relicensing as required by company, local, regional, and state policies, regulations or laws.
· Demonstrates complete knowledge, understanding and compliance of company policies and procedures.
· Ensures the security of all company computer systems by adhering to security procedures and utilizing appropriate passwords policies.
· Ensures security of buildings, vehicles, equipment, supplies, systems and all other company property.
· Acts at all times with his/her personal safety in mind, as well as the safety of others.
· Maintains security and privacy of all company and patient information at all times in accordance HIPPA and all other local, state and federal regulations.
· Maintains security and privacy of all confidential and proprietary company information at all times in accordance with company policy.
· Reports to assigned shifts properly groomed and attired and on time unless the proper notification has been made.
Remains on duty until properly relieved.
· At all times sets an example of professionalism and leadership for others to follow.
· Assures all company goals, objectives and procedures are followed on a daily basis.
· Performs other duties as directed by management.
