Responsibilities:
· Assesses patient/family educational needs and expectations of hospitalizations
· Incorporates clinical indicators (labs, radiology, vital signs) in SBAR hand-off report to physician or other staff
· Assesses the biological, psychological, physical and cultural needs of patients/families
· Recognizes and provides ongoing assessment of any change in patient condition
Skills:
· Graduate of an accredited or NLN-approved RN program.
· Current state nursing license
· Must successfully pass any pre-employment assessment(s) required by the facility.
Required:
