com Disease Specific Clinical Nurse Navigator is to serve as a liaison between the physicians, patients, and staff to ensure the Center delivers care that will minimize complications and maximize patient outcomes related to Stroke, Sepsis, Heart Attack, and Heart Failure.
The Nurse Navigator will assist with tracking the progress of the Disease Specific programs, identifying areas that require improvement and facilitating improvement actions.
The Nurse Navigator will provide patient education and assist with program marketing strategies.
The Nurse Navigator will collaborate with the Manager of Disease Specific Programs, Service Line Directors, and other leaders to improve the overall outcomes of the disease specific care program patients.
Duties: Demonstrates the ability to communicate and implement the hospital’s mission, vision and values as well as departmental goals.
Provides education to the patient population.
Conducts follow up calls to stroke, sepsis, heart failure and heart attack patient population.
Coordinates follow up appointments for disease specific program patients.
Communicates with key stakeholders and various disciplines to improve the care delivery of disease specific program patients.
Participates in performance improvement committees, projects, etc.
as indicated.
Inputs data into disease specific quality registries.
Conducts interrater reliability for data review.
Identifies opportunities, trends, and facilitates process improvement.
Collection and submission of accurate and reliable data into registries, in a timely manner.
Work closely with departments to identify opportunities for clinical quality improvement related to disease specific care.
Work with process improvement leaders to set and achieve goals related to identified opportunities or metrics.
Develop reports to track and trend improvements in procedural outcomes and quality to optimize patient care.
Remains up to date on regulatory and publically reported data requirements.
Regulatory reviews updates/FAQs from registries.
Participates in monthly conference calls with registries and other collaborative to improve data and quality abstraction.
Leverages evidence based best practices to improve care and outcomes of disease specific program patients to reduce adverse outcomes and complications.
Assists with the education of staff.
Develops strategies to promote staff development and competency assessment as related to disease specific care clinical best practice.
Provides support with orientation and training as related to Stroke, Sepsis, Heart Attack and Heart Failure for relevant staff to meet job expectations and competencies.
Participates in community outreach education and program support with Marketing to fulfill our mission as related to the disease specific care programs.
Works together with inpatient units , ancillary services and ED to ensure patient needs are met in a timely manner.
Contributes to overall organization goals and interdisciplinary collaborative initiatives.

Qualifications: Requirements: Education requirements: Graduate of an accredited or NLN approved RN program.
BSN preferred.
Work experience requirements: At least 3 years experience in area of practice.
Required licenses/ certifications/courses Current SC State RN Licensure; BCLS.
Required skills, knowledge and abilities: Proven working relationship with physicians.
Experience with patient education.
Ability to speak in front of a small group.
Excellent organizational skills.
Computer experience with Excel and Power Point is preferred.
